Thank you for offering to help me out with building my portfolio for my Boudoir Photography. Please fill out the form and lets get started on capturing some great shots. **Note; A model release form must be signed prior to the shoot taking place, This release form is a basic formality to which the model (yourself) gives permission to the photographer, the use of the images taken during the shoot to be uploaded to the website.
